Skilled Nursing Care at Home

Skilled Nursing Care, Delivered Where You’re Most Comfortable

Contact Us

A hospital room is built for emergencies, not for daily life. Once the crisis passes, most people do better back in their own kitchen, their own bed, surrounded by the people who know them best. That’s the thinking behind our skilled nursing care at Active Home Health, Hospice & Personal Care.

Whether you’re managing a chronic condition, healing after surgery, or trying to keep a parent safely comfortable at home, our skilled nurses bring hospital-level care straight to your living room. It’s consistent care from a team that’s been serving Utah families for over ten years, backed by a 5-star Medicare rating and a name people in more than seventy cities across five Utah counties have come to trust.

What Is Our Skilled Nursing Service?

Skilled nursing is medical care that draws on a licensed nurse’s training and clinical judgment, well beyond what a caregiver without medical training can safely provide. Our in-home skilled nursing team handles the clinical side of both recovery and ongoing condition management, the parts that need medical supervision, so patients can stay in familiar surroundings instead of a facility bed.

This isn’t the same as our personal care program, which focuses on daily living support like bathing, dressing, or meal prep. Skilled nursing is on the more clinical end of the spectrum. It includes wound care, medication management, monitoring vital signs, and coordinating with your physician on the treatment plan.

Who Can Benefit from In-Home Skilled Nursing

Home nursing care tends to make sense at a few specific points along someone’s health journey. If any of these sound familiar, it might be time to have a conversation with us.

Patients managing chronic conditions like diabetes, heart disease, COPD, or kidney disease.

Seniors whose medical needs have grown a bit more complex than a family caregiver can safely handle alone.

Anyone recovering from a hospital stay, surgery, or a serious illness.

Patients who need consistent monitoring but don’t need to be admitted anywhere.

Families come to us at very different points in this journey. Maybe Dad’s out of the hospital, but nobody’s comfortable managing his wound care solo, or Mom’s blood sugar keeps swinging and the family needs someone watching more closely than anyone is able to. That’s where our skilled nurses come in.

Skilled Nursing FAQs

What is the difference between skilled nursing at home and a skilled nursing facility?

A skilled nursing facility (SNF) is a licensed building where patients stay around the clock, usually after a hospital discharge that requires close, continuous supervision. In-home skilled nursing delivers that same clinical expertise, wound care, medication management, monitoring, and so on, but a nurse visits your home on a scheduled basis rather than you moving into a facility. For patients who don’t need 24/7 supervision, staying home with visiting skilled nursing support often means greater comfort, fewer infection risks, and far less disruption to daily life.

Does Medicare cover in-home skilled nursing care?

In many cases, yes. Medicare covers home health services, including skilled nursing care, when a doctor certifies that you’re homebound and the care is medically necessary and part-time or intermittent rather than continuous. Coverage details can shift depending on your specific plan and situation, so we always recommend a direct conversation with our team to walk through your coverage before care begins.

How often will a nurse visit?

It depends on your care plan. Some patients need a nurse stopping by several times a week during a recovery period, while others managing a long-term condition might see a nurse less frequently, with visits adjusted as circumstances change. Your physician’s orders and your nurse’s clinical assessment shape the schedule, and we revisit that plan regularly rather than locking it in and forgetting about it.
