Hi there. I’m nurse Jane with Active Home Health, Hospice, and Personal Care. I often get asked by family, friends, and neighbors, “What is home health?” and “What are the services it provides?” Below, I answer these frequently asked questions to help you understand home health a little better.

What is Home Health?

Home health services are designed to restore and improve an individual’s health condition or level of function. We focus on restoring a patient’s active lifestyle and independence, all the while helping them remain in their home, and preventing hospitalization or admission to long-term care institutions.

What Can I Expect? What Will Home Health Do to Help Me?

Active Home Health nurses, like myself, manage each individual patient and create a unique custom care plan for everyone! Using one or more of the following focuses we:

We observe and assess when a patient’s condition has changed or is likely to change. We work with their doctor to make adjustments and ensure the adjustments have the desired effects. These include managing abnormal vital signs, weight fluctuations, medication adjustments, behavior changes, and more.

We teach and train patients or their caregiver(s) on the disease process and how to manage the treatments appropriate to their functional loss, illness, or injury. This may include gait and transfer technique training, IV management, ostomy teaching, diabetic teaching, and many other disease-specific trainings

We administer medications when a skilled nurse is necessary to safely and effectively administer the medication for the treatment of an illness or injury. This includes IV infusions, injectable antibiotics, Vitamin B-12 injections, insulin injections, and more.

As your nurse and primary home health manager, I also enlist the help of my team members to provide other non-nursing services. such as:

  • Physical Therapy
  • Occupational Therapy
  • Speech Therapy
  • CNAs
