Who can benefit from home health care?
Home health care is a service that can significantly benefit those looking to improve their health and regain mobility, strength, or independence. You can benefit from Utah home health care services if you:
- Have a chronic illness such as diabetes, heart disease, COPD, or kidney disease
- Are recovering from hospitalization, surgery, or illness
- Need skilled help to manage your condition effectively
- Visit the hospital or your physician often
- Have a considerable risk of experiencing severe illness from COVID-19
How often will I receive home health care?
Your home health care plan is personalized to your individual needs; when care begins, our home health team will work with your existing physician to determine the frequency and nature of your care. They will consider your current healthcare needs, your home environment, and your existing level of support from family or caregivers. As time goes on, plans often evolve to serve you best. For example, the frequency of visits may increase or decrease, or your home healthcare provider may add an additional service to your visit (such as physical therapy).
Who will take care of me while I'm in hospice care?
While you’re in hospice care, our team tends to not only your physical and pain management needs but also your emotional and spiritual needs. Our team of healthcare professionals includes a medical director or physician, skilled nurses, aides, social workers, counselors or other spiritual care providers, bereavement professionals, massage therapists, and trained volunteers. Working to support both you and your family’s needs, this team of specialists is available around the clock and is directed by you and your family’s needs; our team listens to your wishes to make your end-of-life experience as comfortable as possible.
What are personal care services?
Personal care, also known as home care, helps seniors who need assistance with daily living to remain in their own homes. This includes help with eating and meal preparation, dressing, toileting, bathing, grooming, medication reminders, and general mobility. At Active Home Health, Hospice & Personal Care, our Utah personal care aides are fully licensed, trained, and vetted but are not nurses or health care professionals. They can best support those who do not require routine skilled nursing care.

Active’s Purple Tuesday
At Active Home Health, Hospice & Personal Care, we dedicate every Tuesday as “Purple Tuesday” to increase awareness of and education on Alzheimer’s Disease and other dementias. Not only do we wear purple every Tuesday, but we also help spread the word through statistics, tips, and even local competitions. Join us by wearing purple every Tuesday and by following us on social media to learn how you can help. Separate we might be few, but together, anything is possible!
